Diagnostic Significance of Ultrasonography in Acute Appendicitis

Abstract
|
|
|
For evaluation of diagnostic significance of ultrasonography 83 cases were analysed who were diagnosed clinically as acute appendicitis, examined with ultrasonography(method : graded compression, machine : Acuson 128 scanner with 3.5MHz convex
array
prove &/or 5MHz linear array prove) preoperatively, and underwent appendectomy from January 1992 to June 1992 at Maryknoll hospital. The criteria for appendicitis positive in ultrasonographic findings were visualization of appendix and/or
extraluminal
fluid collection around suspected appendix.
@ES The results obtained are summerized as follows :
@EN 1) The overall sensitivity of US diagnosis of appendicitis was 68.5% with a specificity 70.0%. The overall diagnostic accuracy was 68.7%
2) The sensitivity, specificity and accuracy of US diagnosis in sex and age were no significant differences.
3) The sensitivity of US diagnosis in different positions of appendix was 67.5% in retrocecal position, 69.3% in ileal position, 71.4% in pelvic position, 66.7% in retroperitoneal position.
4) The sensitivity of US diagnosis in different abdominal wall thickness was 63.9% in above 4cm wall thickness, 72.9% in below 4cm wall thickness.
5) The sensitivity, specificity, accuracy of US according to US examiner was 72.9%, 77.8%, 73.9% by staff, 63.9%, 0%, 62.2% by resident.
6) The sensitivity of US diagnosis in different pathological types was 11.1% in focal type, 81.3% in suppurative type, 40.0% in gangrenous type, 71.4% in perforative type, 75.0% in periappendiceal abscess type.
We confirmed ultrasonographic examination for early diagnosis and differential diagnosis of acute appendicitis is no longer valid in our institute than the conventional diagnostic methods such as history taking and physical examination unless
advancing
of the ultrasonigraphic technique.
